Reentry, Opportunity, and Workforce Transformation in South‑Central Wisconsin

Expanding Opportunity

The Pathway Home 6 grant supports a coordinated regional effort to help individuals returning from incarceration successfully reconnect with their communities and the workforce. Funded by the U.S. Department of Labor, this initiative focuses on reducing barriers to employment, strengthening economic mobility, and improving long‑term outcomes for justice‑involved individuals across south‑central Wisconsin.

Pathway Home 6 brings together workforce, corrections, community, and employer partners to deliver services before release, at the point of transition, and throughout reentry, ensuring participants have the tools and support they need to build stable, meaningful careers.

Support and Services​​

Participants in the Pathway Home 6 program can access a comprehensive set of services tailored to their goals and needs. These may include:

  • Pre-Release Services: Individualized reentry and employment planning for transition and stabilization

  • Transition and Re-Entry Support: Providing case management services and help with resource navigation and community referrals

  • Employment and Training Services: Job placement help and employer connections, connections to training opportunities and post-employment coaching and retention support

  • Wraparound Supports: Offering comprehensive support services to ensure job seekers and workers thrive in their career journeys.

Services may vary depending on eligibility and availability.
